The State of Florida OMMU has updated the Request for Exception submission process. Patients are now required to submit a Medical Visit Summary from the physician that diagnosed you with the qualifying condition. Once you receive the visit summary of your last visit, we can assist you with submiitting it to the MMUR website. You can upload it via your private patient portal through On-Patient, or you can email it or bring it into the clinic. Don't hesitate to contact us for any questions or concerns.
